Health & Welfare Plans
DUDA’s medical plans are designed to promote good health and protect you and your eligible dependents from financial risks associated with medical conditions, illnesses, or injuries. Prescription drug coverage is provided to all employees enrolled in a health plan. The cost is shared by DUDA and you on a pre-tax basis.
The Dental Plan encourages regular dental care by providing coverage for preventive and diagnostic care, basic and major care, and orthodontics (for adults and dependent children). The cost is shared by DUDA and you on a pre-tax basis.
The Vision Plan offers coverage for annual routine eye exams as well as benefits for eyeglasses, including lenses and frames, or contact lenses through a network of providers. The cost is shared by DUDA and you on a pre-tax basis.
Flexible Spending Accounts
These plans allow employees to set aside pre-tax dollars by payroll deduction to pay for certain health and dependent care expenses, as allowed by the IRS.
Basic Life Insurance
Life insurance benefits in the amount of 1.5 times annual base earnings to a maximum of $100,000 are provided to all full-time employees. This benefit is provided at no cost to employees.
Short-term disability benefits are provided to all full-time employees at no cost. This coverage provides short-term disability benefits to protect against income loss if you cannot work because of a qualifying temporary illness or injury. The benefit covers 66 2/3% of weekly base earnings for up to 180 days of physician-certified medical leave.
Long-term disability benefits are provided to all full-time, salaried employees at no cost. This coverage provides protection if you are determined by the company’s insurance carrier to be disabled for a period of more than 180 days.
You can contribute from 1% to 50% of your covered pay through a traditional pre-taxed deferral and/or a post-taxed Roth 401(k) option through convenient payroll deductions. Your combined contributions are subject to the annual limits set forth by the IRS. DUDA provides a matching contribution equal to 100% of the first 3% of your covered pay that you contribute to the Plan, plus 50% of the next 2% of your covered pay that you defer and contribute to the Plan. You also may be eligible for a quarterly discretionary, non-elective contribution made by the Company based on your years of service and your covered pay.
Optional Term Life Insurance
Optional term life insurance is available through payroll deduction. Premiums are based upon amount of coverage and age. Coverage also is available for spouses and dependent children.
Whole Life Insurance with Long Term Care Rider
Coverage is available for whole life insurance with a long-term care rider to help pay for nursing home or assisted living facilities under certain circumstances. This coverage is available through payroll deduction. Premiums are based upon amount of coverage and age.
Accident insurance pays a set amount direct to the employee for “off the job” accidents/injuries. Premiums are paid through payroll deductions. Both individual and family coverage is available.
Critical Illness Insurance
Critical Insurance pays benefits directly to the employee for certain critical illnesses and on initial diagnosis of certain conditions. Premiums are paid through payroll deductions. Coverage is available for employees and dependents.
Cancer Select Plus
A cancer indemnity product which pays benefits directly to the employee in the event of a cancer diagnosis. Premiums are paid through payroll deductions. Coverage is available for employees and dependents.
Life Lock Advantage
Identity Theft protection covers the employee and provides security to personal account information. Premiums are paid through payroll deduction. Coverage is available for employees and dependents.
Please note that these benefit descriptions are intended to highlight the programs and benefits currently offered. These plans are described in greater detail in summary plan descriptions (“SPDs”) and official plan documents. In the event of any conflict between these descriptions and official plan documents, the official plan document will govern.
DUDA retains full discretionary authority to interpret the terms of each plan and to address administrative matters arising in connection with the plans. DUDA may, at its sole discretion, modify or eliminate any benefits or programs it currently provides, as well as increase the cost to employees of such benefits and programs. This discretionary authority extends to all issues concerning benefit eligibility and entitlement.
